CYCJ responds to the FAI into the deaths of William Lindsay and Katie Allan

The Children and Young People’s Centre for Justice (CYCJ) has welcomed the publication of the Fatal Accident Inquiry (FAI) into the tragic deaths of William Lindsay and Katie Allan at HMP&YOI Polmont.

Led by Sheriff Simon Collins KC, the findings of the FAI highlight multiple failings and conclude that both of these incidents could have been avoided. The report sets out numerous recommendations to ensure these events are never repeated.

Fiona Dyer, Director of CYCJ: “The thoughts of everyone at CYCJ are with the families. Both these terrible tragedies could have been avoided. We urge all the agencies involved to prioritise meeting the recommendations in full to ensure nothing like this every happens again”
